Claims Process

For each case the insured must complete a claim form. The original invoices, a detailed report on the claim form and any related medical records must be sent along with the claim form. The medical records should include details of diagnosis, date of first symptom, time of visits and type of treatment.

 

Pre-Notification

1. Hospitalization programmed in any part of the world.

2. Hospitalizations or emergencies must be notified in the first 48 hours following the admission to the hospital or as soon as it is reasonably possible.

3. Ambulatory surgery in any part of the world.

4. Transplant in any part of the world.

5. Any procedure that exceeds $1,000

– Pre-notification forms can be sent by e-mail to medical@claria.us. Claria will coordinate all hospital admissions.

– The Explanation of Benefits is processed within 7-10 business days of receiving all the necessary documentation.
